About Dr. Trock

Dr. Joshua Trock, PT, DPT, CWS, RVT, CLT-LANA

Diplomate, American Board of Wound Management

Vascular, Lymphatic, & Advanced Tissue Restoration Specialist

The Evolution of My Practice

My journey in physical therapy did not follow a conventional linear path. It began in the trenches of traditional clinical care and home health operations in South Texas, where I quickly observed a significant, systemic gap in how chronic leg swelling, vascular abnormalities, and complex tissue issues were managed.

Patients searching for the best lymphedema therapist in my area or an expert to diagnose severe, life-altering lower extremity swelling and non-healing wounds were routinely shuffled between multiple disconnected medical silos. They were subjected to a constant cycle of misdiagnoses—often told their structural leg changes were simply "weight gain"—or sent down dangerous rehab paths like using compression pumps or blood flow restriction (BFR) training when an active deep vein blood clot (DVT) had not been properly ruled out.

Seeing this continuous fragmentation forced a definitive pivot in my career. I realized that to truly protect patients, serve as the premier specialist for leg swelling in San Antonio, and eliminate the clinical guesswork holding back recovery, a physical therapist needed to combine elite tissue management with advanced macro- and microvascular diagnostics.

Constructing the "Unicorn" Framework

I committed my post-doctoral career to systematically capturing the highly distinct board certifications required to unite these fields into a single clinical lens. By building upon my foundational Bachelor’s Degree in Biology from the University of Texas at San Antonio (UTSA) into my dual degrees of Bachelor’s in Health Care Science and  Doctorate in Physical Therapy from the University of Texas Health at San Antonio, I secured a rare, specialized clinical matrix uniquely qualified to treat complex swelling disorders:

  • Vascular Diagnostics (RVT): Credentialed as a Registered Vascular Technologist through the APCA to master macrovascular ultrasound imaging and map underlying venous insufficiency, venous hypertension, tissue abnormalities, and deep vein thrombosis directly at the point of care.
  • Advanced Lymphology (CLT-LANA): My initial CLT was early in my career through Klose Training and Consulting. I then achieved the highest tier of national board certification through the Lymphology Association of North America (LANA) to show mastery in navigating complex lymphatic fluid mechanics, lipedema tissue remodeling, and microvascular transport failures.
  • Tissue Restoration (CWS): Earning my board certification as a Certified Wound Specialist to serve as a Diplomate of the American Board of Wound Management, standardizing specialized interventions for advanced tissue breakdown and chronic ulcerations.
  • Geriatric Performance and Recovery (CEEAA & CEWS): Layering my distinguished certified expertise in exercise management for aging adults through the American Board of Geriatric Physical Therapy  and specialized edematous wound care (such as lymphedema compression with simultaneous Wound VAC use) from the Norton School of Lymphatics.

My Mission Today: Leadership, Disruption, & Human Flourishing

Today, I refuse to look at fluid transport, lipedema fat, or advanced wound care through a single, isolated perspective. My career spans high-level clinical operations management, optimizing practice profitability, and lecturing nationally as a vascular and lymphatic subject matter expert.

I operate as a deliberate thought leader and disrupter in the rehabilitation space, actively questioning established norms and cutting through the traditional medical dogmas that have left chronic swelling patients underserved for decades. By introducing a cutting-edge, novel approach that unites point-of-care macrovascular ultrasound imaging with microvascular lymphology, I am rewriting the standard clinical playbook.

Whether I am presenting clinical masterclasses, building the advanced primary care vascular curriculum for peer organizations, or publishing case-study breakdowns on my media channels, my mission remains entirely the same: To eliminate clinical guesswork, cut through the noise of medical misinformation, and equip both clinicians and patients with the exact, evidence-based tools they need to achieve permanent long-term management. Every piece of curriculum I build and every clinical framework I design is engineered to optimize care delivery, accelerate patient independence, and ultimately promote true human flourishing.
